Welcome to Genesis Physical Rehabilitation at Valley Fair. We specialize in offering programs that meet the needs of patients requiring physical, occupational and speech therapies. Treatment

Outpatient therapy is available for patients who have moderate to severe physical limitations and who can travel to receive care. Back, soft tissue, neurologic, lymphedema and work related injuries are the most frequent ailments referred to outpatient therapy.

Other ailments commonly treated in an outpatient setting include:

  • Spinal dysfunction

  • Pain from motor vehicle accidents or other trauma
  • Cumulative trauma disorders such as carpal tunnel syndrome
  • Hand injuries, surgeries or splinting needs
  • Joint replacement and revisions, (Most common total hip or knee surgeries)

To meet our goal Genesis Physical Rehabilitation at Valley Fair provides our patients with the most up-to-date therapeutic services and help the patient return to either their prior level of functioning or to increase their level of functioning. The center provides outpatient physical therapy, outpatient occupational therapy and outpatient speech therapy. All of the services are provided by highly educated physical therapists, occupational therapists, physical therapy assistants, occupational therapy assistants and speech therapists.  All of our therapists stay abreast of the latest techniques and technology in their respective fields by regularly attending continuing education courses. The therapists are supported by a friendly staff of administrative assistants.

Please allow our facility to serve all of your outpatient physical therapy, outpatient occupational therapy and outpatient speech therapy needs. We offer convenient hours from 7:00 a.m. to 6:00 p.m. Monday through Thursday and 7:00 a.m. to 4:40 p.m. on Friday with other hours arranged as needed. Mary BakerMary Baker, DPT

Title: Physical Therapist and Clinic Supervisor for the Valley Fair Location

School and Year of Graduation: Chicago Medical School/University 1979, B.S., St. Ambrose University 2013

Began with Genesis: 2007

Specialization/Certification: Mary has treated in a variety of settings and has served as a supervisor for over 27 years of her career. She practices as a generalist. Her experience includes inpatient, outpatient, long term care, home health, hospital based rehabilitation, school based therapy, work hardening, and functional capacity testing with an emphasis in rural settings. Mary is a certified Brain Injury Specialist and has been trained in the use of BioNess for neuromuscular re-education of the ankle to address foot drop. She has a keen interest in documentation and coding for proper billing. Mary’s areas of interest for her practice lie with MS, balance, amputation, and practicing within the outpatient setting.

Community/Recreational Activities: In her free time, Mary enjoys. knitting, cooking,  skiing, hiking, SHIIP, and being active in her local church, SHIIP

Liz ArpLiz Arp, DPT, C/NDT

Title: Physical Therapist

School and Year of Graduation: St. Ambrose University 2006, DPT

Began with Genesis: 2007

Specialization/Certification: Liz enjoys working with a variety of patients with orthopedic and neurologic conditions, including patients with stroke, brain injury, spinal cord injury, parkinson's, MS, and vestibular conditions. Liz received NDT certification in 2012 and completed a vestibular competency course in April 2015. Liz is trained in performing Functional Capacity Evaluations and in utilizing BioNess. Liz is an APTA certified Clinical Instructor for PT students.

Community/Recreational Activities: Liz enjoys spending time with her husband and 2 dogs, going on walks, and playing volleyball.

Cara Gates

Cara Gates, DPT

Title: Physical Therapist

School and Year of Graduation: University of Iowa 2009, DPT

Began with Genesis: 2010 

Specialization/Certification: Cara is a generalist that treats patients with a variety of orthopedic and neurologic impairments. She also enjoys treating patients with dizziness, amputations, or balance deficits. She is trained in the use of BioNess L300 Plus, a walking aide for persons with neurologic weakness of the legs. 

Tammy Conner

Tammy Conner, OTR/L CLT CEAS

Title: Occupational Therapist

School and Year of Graduation: St. Ambrose University 1995, OT and B.S. in Psychology

Began with Genesis: 2001

Specialization/Certification: Tammy has completed graduate work in the area of Behavioral Science. During her career she has become a certified lymphedema specialist and trained as an Ergoscience FCR provider. Tammy has an interest in treating patients with upper extremity orthopedic injuries, splinting, burns, occupational health, and lymphedema. She is trained in the use of BioNess for neuromuscular re-education and has been certified as an Iserhagen provider for FCE's, work hardening, worksite evaluation, and job site analysis, Tammy has also gone through training to become a Certified Ergonomic Assessment Specialist (CEAS 1).

Community/Recreational Activities: Tammy enjoys swimming and is an advocate for individuals diagnosed with Autism Spectrum Disorder.

Rebecca Rall

Rebecca Rall, MA, CCC-SLP

Title: Speech and Language Pathologist

School and Year of Graduation: University of Northern Iowa 1987, MA

Began with Genesis: 1998

Specialization/Certification: Rebecca has experience in inpatient and outpatient rehabilitation, long term care, home health, pediatrics, pulmonary care, neonatal care, and consultant for group homes for the developmentally delayed. Rebecca currently specializes in stroke and brain injury recovery and oral myofunctional therapy. Rebecca currently holds certification in Interactive Metronome, is trained in use of the IOPI for oral motor recovery, and has interest in evaluation and treatment with Augmentative Communication devices.

Allison ThompsonAllison Thompson, MOTR/L, OT

School and Year of Graduation: St. Ambrose University 2005, MOTR/L, CLT
Title: Occupational Therapist

Began with Genesis: 2005

Specialization/Certification: Allison is trained in the use of BioNess H200 for upper extremity neurologic dysfunction. She enjoys working with a variety of patients with stroke, brain injury, and upper extremity orthopedic injuries. Allison also specializes in the treatment of patients with lymphedema.

Brooke Ketner

Brooke Ketner, PTA

Title: Physical Therapy Assistant

School and Year of Graduation: St. Ambrose University 2004 and Blackhawk College 2006, AAS

Began with Genesis: 2006

Specialization/Certification: Brooke graduated from St. Ambrose University with a Fitness and Human Performance Degree in 2004. Since she enjoyed working with post surgical patients in an orthopedic realm she decided to pursue an AAS Degree as a Physical Therapist Assistant. Brooke graduated in August 2006 with a PTA Degree from Black Hawk College and now enjoys working with a variety of patients. Orthopedic patients and sports performance athletes are two areas that interest Brooke the most.

Heather VanCamp

Heather VanCamp, PT, DPT

Specialization/Certification: Heather enjoys working with orthopedic patients of all ages.

Community/Recreational Activities: Heather enjoys spending time with family, boating, swimming, and volleyball. Heather also enjoys cheering on the Cubbies and Hawkeyes.


Ashley Odell

Ashley O'Dell, PT, MPT

Title: Physical Therapist

School and Year of Graduation: Northern Illinois University 2010, MPT

Began with Genesis: 2010

Specialization/Certification: Ashley graduated from Northern Illinois University in 2010. She specializes in orthopedics, but has a specific interest treating patients with total joint replacements and back pain.

Community/Recreational Activities: In her free time, Ashley likes to scrapbook, spend time with her family, and go for walks. 

Shelly ZahnShelly Zahn, PT, MPT

Title: Physical Therapist

School and Year of Graduation: Northern Illinois University 2007, MPT

Began with Genesis: 2007

Specialization/Certification: Shelley graduated from Northern Illinois University in 2007.  She specializes in treating all types of orthopedic conditions and post-surgical recovery.  She enjoys working with patients of all ages.  Shelley also completes functional capacity evaluations (FCE) for return to work.

Community/Recreational Activities: In her spare time, Shelley loves to spend time with her family, travel, run, and cheer on the Chicago Cubs.

Mary GordonMary Gordon, MOTR/L

Title: Occupational Therapist

Education: Mary graduated from St. Ambrose University with 

her Masters in Occupational Therapy in 2010.

Began with Genesis: Fall 2013

Specialization/Certification: Mary began her career practicing in a hospital setting in Kewanee, IL providing care to pediatric, adult, and geriatric patients in a variety of settings. Mary has furthered her therapeutic knowledge by attending classes on topics including orthopedics (Wisconsin Hand Experience), the use of thermal and electrical physical agent modalities, Splinting, multi-sensory strategies and tools, accessible play via toy adaptation, self-regulation in children/The ALERT Program, and treatment of children with sensory, emotional, and motor challenges. Mary is also certified to utilize Therapeutic listening and the SOS approach to feeding. As an occupational therapist, Mary provides individualized care to increase independence for all ages.

Community/Recreational Activities: In her spare time, Mary can be found spending time with family and friends, reading, gaming, baking, or singing karaoke. 

Kami HolstKami Holst, MOTR/L

Title: Occupational Therapist

School and Year of Graduation: St. Ambrose University 2014

Began at Genesis: 2014

Specialization/Certification: Kami enjoys treating patients with various upper extremity orthopedic injuries, splinting, burns, occupational health patients. Kami is also on staff as a part of the LIFT cognitive rehabilitation program and has an interest in treating brain injury and stroke patients. She is trained in the Bioness H200 system for neuromuscular rehabilitation for the upper extremity. 

Community/Recreational Activities: Kami is passionate about fitness/health & wellness. Kami enjoys spending her free time as a Zumba instructor.

Rusty PlunkettRusty Plunkett

Title: Physical Therapist

School and Year of Graduation: University of Iowa, 2010

Began with Genesis: 2013

Specialization/Certification: Rusty graduated from the University of Iowa in 2010 with his Doctorate of Physical Therapy degree. Since that time, he has developed an interest in treating patients of all ages with orthopedic injuries. He has a special interest in treating patients with chronic pain, neck pain, and low back pain. He also enjoys working with athletes of all ages and works collaboratively with JP Sports, a premier youth baseball tournament organizer in the Quad Cities Area. 

Community/Recreational Activities: In his free time, Rusty enjoys spending time at home and laughing at re-runs of Ridiculousness on MTV. He stays active by playing basketball on Thursday nights in an old man's pick-up game (he likes to call this "job security.") In the summer, you will likely find him taking in a baseball game at Modern Woodmen Park, Wrigley Field, or in his hometown of Viola.
