Student Job Shadow Program
Welcome to the Genesis Job Shadowing Program. We're glad you are here! This experience will be a glimpse into the health care career you have in mind for the future.
Please take a close look at all the form checklist below. ALL FORMS MUST BE COMPLETED AND RETURNED BEFORE YOUR SHADOW APPLICATION IS COMPLETE. Jobs shadows happen every Tuesday, Wednesday and Thursdays from 1:00pm-3:00pm.

Form Checklist:
- Please read the Student Shadow Program Policies.
- Student Information Sheet
- Health Care Career Interest (Number 1-3 in order of preference)
- Affirmation and Release of Liability (18 and under or 18 and over)
- Release of Immunization Record (Attach a copy of your immunization card)
- School Counselor Recommendation (School counselor signature)
- Understanding of Health Care Confidentiality
- HIPAA Exam: 15 questions and essay (Must pass in order to shadow)
- Dress Code
Once these forms have been completed and returned, the AHEC office will correspond with you via email and provide you with the date, time and location of your job shadow.
