Scholarship History & Overview
The Patricia V. Samuelson Memorial Scholarship Fund was established in 1988 by John L. Samuelson in loving memory of his wife, Patricia, who passed away in 1986. Jack established the Patricia V. Samuelson Memorial Scholarship Fund for students in the field of respiratory therapy. A life-long resident of this community, Jack was retired from the John Deere Foundry. He served as a member of the Illini Hospital Foundation Board of Directors, and was active on the Black Hawk College Respiratory Care Committee. Jack was a resident of Illini Restorative Care when he passed away August 26, 2009.
The purpose of the Patricia V. Samuelson Memorial Scholarship Fund is to educate students in the field of respiratory therapy by providing scholarships to new students in the field, grants for credentialing and licensing to existing GMC, Silvis Respiratory Therapy employees, or other GMC Respiratory Therapy employees that rotate through or work part time at GMC, Silvis as well as provide funding for other continuing education programs for the GMC, Silvis Respiratory Therapy staff members.
Awards for scholarship and grants may be presented annually. Awards will be made for scholarships at educational institutions in Western Illinois or Eastern Iowa providing for programs and educating students in the field of respiratory illnesses. Credentialing and licensing grants will be rewarded as reimbursements to the employee.
Endowment Administration and Funding
Funds will be managed and allocated in accordance with general policy of Genesis Health Services Foundation endowment funds.
Awards from this fund may include grants for certification and credentialing as well as scholarships for degree programs. Up to one-half of the annual allocation of this fund may be used for educational programs regarding health care, seminars, classes, exhibits and other health related activities sponsored by Genesis Medical Center, Silvis.
Criteria for Scholarship to Area Students
- Applicant may or may not be employed by Genesis Medical Center, but if not an employee, must be a resident of the Genesis Health System multi-county service area.
- Applicant must be enrolled full time in a two- or four-year program in the field of respiratory therapy at an accredited college or university in Eastern Iowa or Western Illinois. Applicant must have graduated in good academic standing (3.0 or above) from an area High School and maintain good academic standing while in college (3.0 or above.)
Criteria for Credentialing Grant to GMC, Silvis and other GMC Employees
- Applicant must be employed at Genesis Medical Center, Silvis or be employed by another GMC entity but rotate through or work at least part time at the Silvis campus.
- Applicant must exemplify the mission and values of Genesis Medical Center, Silvis. This will be validated by a supervisor’s signature on the completed application.
Application is to be completed online. Supporting documents should be uploaded with the application. Only a complete application packet can be submitted.
The Patricia V. Samuelson Scholarship Committee shall include the Manager of Respiratory Services, GMC, Silvis, the Director of Finance, GHS, and the Executive Director of the Genesis Health Services Foundation or an appointed representative. The President of GMC, Silvis will approve all scholarships and grants.
The Patricia V. Samuelson Scholarship Committee will meet annually to select recipient(s).
The award selection will not be made on the basis of race, color, religion, national origin, sex, ancestry, marital status, unfavorable military discharge, handicap or age. No question on the application is intended to secure information to be used for such discrimination.
Scholarship recipients will be required to submit a summary of experience to the Genesis Health Services Foundation Board.
