Jackson County Health Foundation Golf Outing
The 28th Annual Jackson County Health Foundation Golf Outing is scheduled for Friday, June 14th, 2013 with a 2:00 p.m. shotgun start at the Bellevue Golf Club. For those who have participated in the past, we thank you, for those who are new, we welcome you.
This year our goal is to raise $35,000 for the ambulance service. We are sure as you reflect on the past year; you will be able to think of a story or event that involved the JCRHC ambulance. This service is definitely an essential component for our community, however, with decreased governmental reimbursement, maintaining a financially sustainable service is a challenge; therefore we continuously look for ways to offset the deficit.
We have five levels of sponsorship packages, as detailed on the sponsorship options form. You may select the level of participation that best meets your needs. Please know that your sponsorship fee will be used to help the Jackson County Health Foundation reach our goal to help maintain our ambulance service for the community. If you choose to sponsor a hole, please complete the agreement form and remit payment by May 10, 2013. We are also accepting cash donations and prizes for the golfers. All contributions are greatly appreciated and add to the success of our event.
To play golf, (four-person best shot) please complete the reservation card and remit payment by May 10, 2013 to: Jackson County Health Foundation; 700 West Grove Street; Maquoketa, IA 52060. Your reservation includes nine holes of golf, 2 complimentary beverage tickets, sandwiches on the course, followed by a delicious grilled rib-eye dinner and a chance to win many great prizes.
