Pulmonary Rehabilitation 

Pulmonary Rehab Phase II


  • Individuals who have a history of pulmonary diseases such as Asthma, Chronic Bronchitis, Lung Cancer or Chronic Obstructive Pulmonary Disease (COPD)
  • Any person who has breathing difficulties


  • Phase II Pulmonary Rehabilitation can begin whenever the client is medically stable for an exercise program.
  • The program is an 8-12 week exercise and education program meeting on Tuesdays and Thursdays.
  • Instructions by multidisciplinary team including dietitian, pharmacist, physical therapist and registered nurse are given each session
  • Education sessions include disease management, stress management, home exercise, pacing activities, medications, diet and energy conservation

Entrance Requirements:

  • Commitment to smoking cessation
  • Pulmonary function testing
  • Chest x-ray
  • Exercise testing
  • Physician referral

Cardiac and Pulmonary Rehab Phase III


  • Anyone with a history of heart disease
  • Anyone who has completed the Pulmonary Rehabilitation Program
  • Anyone seeking a fitness program who may not be appropriate for the Fitness Center


  • Minimum of 4 weeks of supervised exercise
  • Unlimited participation in program
  • Upon determination of medical stability, participant may utilize the JCRHC Fitness Center with physician approval

Entrance Requirements:

  • All participants require a physician referral to enter this program.

Cardiac Patient:

  • Yearly stress test
  • Resting EKG

Pulmonary Patient:

  • Yearly Pulmonary Function test
  • Yearly chest x-ray
  • Walking test
