The Affordable Care Act requires everyone legally living in the U.S. to have health insurance beginning January 1, 2014. If you do not have health insurance, you are required to sign up during open enrollment, from October 1, 2013, to March 31, 2014. You can shop for plans by phone, through a broker, at on-site enrollment locations, or at the online Health Insurance Marketplace. If you choose not to enroll for insurance, the law states that you will face tax penalties.

Health Insurance Marketplace

An important part of health reform is the health insurance marketplace. The marketplace will be an online resource for consumers to visit, compare different health plans, and then select which one fits their family's needs and their budget correctly. If your family earns between 138-400% of federal poverty level then you will also qualify for a subsidy, in the form of an advanceable tax credit, to help you purchase insurance.

If you are an Iowa resident, and if your income is between 101-138% of federal poverty level, you will be able to select a plan from Iowa's health insurance marketplace and have your premiums paid by the Marketplace Choice Plan. The Marketplace Choice Plan is Iowa's alternative to the Medicaid expansion originally proposed in the Affordable Care Act.

Who shops the Marketplace?

  • People eligible for Iowa Health and Wellness Plan. Sign up beginning Oct. 1, 2013 and coverage begins January 1, 2014.
  • IowaCare patients. You MUST enroll in the Iowa Health & Wellness Plan by December 31, 2013 to continue coverage.
  • Self-insured individuals looking for commercial insurance.
  • Individuals with no insurance.
  • Small businesses with 1 to 50 employees.

Medicaid Expansion

Medicaid Expansion

Under the original version of the Affordable Care Act (ACA), Medicaid was supposed to expand to cover virtually everyone who made 138% of or below federal poverty level. Opponents of the ACA brought the entire law before the Supreme Court, claiming it was unconstitutional. But on June 28, 2012, the Court ruled the ACA was constitutional in all parts, except the requirement for states to expand their Medicaid programs. This gave states the choice as to whether they would expand their Medicaid programs or not.

Illinois Governor Pat Quinn accepted Medicaid expansion in his state, and signed the bill expanding the program for his state on July 22, 2013. This expanded Medicaid coverage to approximately 342,000 Illinoisans. Medicaid expansion was fully accepted in Illinois, covering the entire qualifying population below 138% of federal poverty level.

Iowa Governor Terry Branstad rejected the original Medicaid expansion. Opponents and supporters rallied for months around the issue during the 2013 legislative session. In May, the Iowa Health and Wellness Plan was proposed, and it passed both houses of the legislature and was signed by the Governor on June 20.

Iowa's plan is very different from Illinois's. The Iowa Health and Wellness Plan only covers those who earn up to 100% of federal poverty level. Those who are on the Health and Wellness Plan who earn more than 50% of federal poverty level will pay a very small monthly premium, between $10-16. This small contribution may be waived if the participant completes health improvement behaviors. Contributions are waived for the first year (2014) of the existence this program.
