The Quad City Health Initiative (QCHI) is a community partnership in active pursuit of a healthier community. The Initiative acts as a catalyst for improving the health and overall quality of life within the Quad City community. The Initiative raises awareness of health issues, encourages or creates programs to address gaps in health services and fosters community collaboration in all aspects of health.

The Health Facts Card Campaign
The Quad City Health Initiative wants every individual in the Quad Cities to be the center of their health care team. The wallet-sized
Health Facts Cards provide individuals with space to record their medications, vitamins, herbal and nutritional supplements, allergies, health history and emergency contact information. Free Health Facts Cards are available at Genesis and Trinity sites as well as at physician offices and pharmacies throughout the community.

The Community Vitality Scan is a joint project of the Quad City Health Initiative, United Way of the Quad Cities Area, Community Foundation of the Great River Bend, The Moline Foundation and The Amy Helpenstell Foundation. The purpose of the scan was to gather the critical information and indicators on community resources and needs to guide community organizations in setting priorities, developing strategies, and allocating resources that will improve the quality of life in our area.
The Community Vitality Snapshot is a report of selected key indicators that we plan to release annually. Our intent is to encourage information sharing, provoke discussion and facilitate collaboration.
