Wig and Wig Care Kit  -OR-  Caps and/or Scarves for Persons Diagnosed with Breast Cancer

The Voucher Program, funded by a grant from the Quad Cities Affiliate of Susan G Komen for the Cure and administered by Genesis Cancer Care Institute, Davenport, Iowa, pays a maximum amount toward the purchase of a wig and a wig care kit (shampoo, spray, wig form, head bands, caps, brush, comb) ordered thru tlc®, (tender loving care) a division of American Cancer Society Products, Inc. Caps or scarves may be substituted for the wig and care kit.

You MUST contact the Voucher Program Office PRIOR to purchasing a wig. The Voucher Program will arrange an individual fitting and wig care consultation services.

If you do not have insurance or if your insurance does not cover the cost of a cranial prosthesis for chemotherapy induced hair loss, please call 563-421-1913 or 1-888-532-7140 to obtain a voucher.
