COVID-19 Vaccination Information for your Scheduled Appointment

Screening Questions:

  1. Are you currently experiencing symptoms of COVID-19 (Fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, or diarrhea)?
    If Yes - You can schedule your vaccine after you have finished your quarantine period.

  2. In the past 14 days, have you tested positive for COVID-19?
    If Yes - You can schedule your vaccine after you have finished your quarantine period.

  3. In the past 90 days, have to received infusion, blood, or plasma as a treatment for COVID-19?
    If Yes - You can schedule your vaccine 90 days from your last COVID-19 treatment.

  4. Have you received any other vaccine within the last 14 days?
    If Yes - You can schedule your vaccine 14 days after you received your last non-COVID-19 vaccine.

  5. Have you ever had an anaphylactic reaction?
    If Yes - You may schedule your vaccine now. You will be asked to stay at the site for observation for 30 minutes after you receive your COVID-19 vaccine.

Appointment Instructions:

  • Arrive 15 minutes before your scheduled appointment time.
  • Bring your driver's license and insurance card.
  • Don't get vaccinated on an empty stomach: Some who have arrived for their vaccination on an empty stomach have experienced nausea.
  • You must wear a mask to the vaccination appointment.
  • Don't be a vaccine no-show. Every drop of this vaccine is needed. Please keep your appointment or cancel 24 hours in advance by emailing
  • The vaccination is a shot in the arm and takes just seconds to administer.
  • Each person is required to stay for 15 minutes in the exam room after administration to be observed for any initial side effects.
  • Vaccinations will be charted in your medical record and exported to the state databases IRIS and ICARE for tracking.
  • Your second dose will be scheduled during the first vaccination appointment and scheduled out as per CDC and vaccine manufacturer guidelines.
  • Vaccination MUST be at same location where the first dose was administered for state COVID vaccine tracking. If in Scott County, all first and second doses will be at the new Genesis vaccination clinic at the former Dick's Sporting Goods store at 5250 Elmore Ave., Davenport.
    COVID-19 vaccines are not interchangeable, and a total of two doses by the same manufacturer will be needed to complete your vaccination.
  • Vaccine preference is not available. It is not known how much vaccine Genesis will receive and from which manufacturer, so we are unable to offer a vaccine preference.
  • If you have an adverse reaction, please seek medical attention from your PCP or Convenient Care. The vaccination clinics will have first aid care available, including EpiPens®. In case of an emergency, 911 will be called.

We recommend you still mask up and social distance after receiving the COVID vaccination. As the vaccine doesn't guarantee complete immunity, you might infect others with COVID-19. It also will take time to achieve herd immunity.
